The Exercise Referral scheme is a great way for people with long term health conditions to increase physical activity levels and improve health. Your GP will be able to refer you to the scheme provided that you are inactive, have a long standing health condition, and/or have a BMI over 30.
All patients are required to attend an Initial Assessment at week 1, followed by a Review Assessment at week 12, in order to measure their progress. Patients will also be given ongoing support to help them increase their weekly activity levels at 2 of the participating leisure centres.
