Poole Intermediate Care Service (PICS) is a joint service for adults and older people in the Borough of Poole designed to:

  • prevent unnecessary admission to hospital/residential care
  • facilitate rapid discharge from hospital

It is funded by the Borough of Poole and Dorset Health Care University Foundation Trust.

This service may include Senior Nursing and Social Work staff undertaking assessments, providing treatment and devising ongoing care packages; with short-term front line practical support provided by a team of Community Rehabilitation Assistants. In practice an older person might receive support to dress, wash, cook, take medication and/or mobilise, in addition to nursing tasks appropriate to their condition. 

Referral to the Service is via the Single Point of Access by the PICS team.
