How the scheme works

The Poole Adult Placement Scheme/Shared Lives provides:

  • short term care for adults with a learning disability to provide a break for families or carers. Care can be for a maximum of 2 weeks at any one time and is agreed within the care plan guidelines
  • long-term accommodation for between 1 to 3 adults with a learning disability in the shared lives provider’s home
  • care is provided by people who are approved by Poole adult social services as shared lives providers for either overnight or long-term care
  • care can be provided in the shared lives provider’s own home

The scheme recruits, assesses, approves and trains people to be carers for adults with learning disabilities. The carers undergo a full assessment over a 6 month period (however it may take longer).

It aims to be informal with the shared lives carer and their families having a flexible approach, which allows them to make their own arrangements within the service specification and care plan limits.

The scheme is available to adults:

  • over the age of 16 years who have a learning disability
  • eligible to receive social services care under our eligibility criteria
  • where a need for shared lives is identified following a community care assessment

Shared Lives handbook

Shared Lives guidance has been developed over many years and has been informed by practice within Shared Lives schemes throughout the UK. The various documents contained in the Shared Lives handbook are not a catalogue of absolute requirements but are intended to help carers and shared lives staff to maintain a consistently high standard of practice and to ensure that they meet the regulatory requirements of the scheme.

Who can be adult placement / shared lives carers

All kinds of people are needed to help all kinds of adults and their families. Shared lives carers can be families (with or without children), grandparents, single people of either sex, single parents, from different cultures, working or not working. They may have had personal experience of disability or have applied interest and want to help in some way. Previous experience is not necessary as training and the opportunity to attend work experience is provided.

The scheme needs people who are sensitive to the needs of adults with learning disabilities and their parents, with the ability to relate to them.

How links are made

Adults with learning disabilities are just as varied in their personalities and interests as the rest of the population. They may have varying needs requiring additional support, and specific training will be provided.

The scheme takes great care to link the service user to the most appropriate carer, taking into account their interests, background, lifestyle, personal circumstances, experience and availability of the carer. The social worker who made the link will be available to offer any help the service user may need.
