When and where to register a death
You must make every effort to register a death within 5 days, unless a coroner is conducting an investigation.
If the person died in Poole, then you will need to call at Poole Register Office to register the death.
You need to book an appointment with us to register a death once you have obtained the medical certificate from the doctor or been advised to do so by the local coroner.
If the death happened outside the Borough of Poole area, you can come to our offices and we'll send a declaration to the relevant office for you. However, please note that this will delay the issue of the relevant forms and certificates to you.

Who can register a death
You can register the death if you're:
- a relative
- someone present at the death
- the person arranging the funeral (not the undertaker)
- if your circumstances are complicated, please telephone or email us for advice
What happens at the Register Office
We will see you in private and ask you straightforward questions about the person who has died. The registrar will create the register entry. We will ask you to carefully check the information on the register and sign that it is correct.
You will need to bring:
- the medical certificate showing cause of death from the doctor or hospital
And if you have it:
- details about any pension or allowance they got from public funds
- their NHS medical card, if available
- their disabled person's parking blue badge, if they had one
What we need to know
You'll need to be able to tell the registrar the following about the person who has died:
- date and place of death
- full name
- maiden name, if applicable
- date and place of birth
- occupation and address
- if they were married or in a civil partnership, the full name, occupation and date of birth of their spouse or civil partner
- your name and address
What you'll receive
We'll give you a document known as a ‘green form’ to give to the undertaker to allow them to make the funeral arrangements for you. If you’re not planning to use the Tell Us Once Service, you'll also be given a form for social security purposes. These are both free of charge.
It is likely that you may need copies of the death certificate to help sort out the affairs of the person who's died, for example, dealing with probate, bank accounts or insurance. You can buy copies of the certificate from the Register Office at any time.
From 16 February 2019 there will be an increase in certificate fees:
- express service - £35
- all other certificates - £11
This increase is a statutory fee from central government and the registrar general.
You can pay by cash, debit or credit card.
Tell Us Once Service
Tell Us Once is a service that lets you report a death to many government organisations when you register a death.
Our Registrars will undertake the service with you in full at the time of your appointment provided you have brought the National Insurance number of the deceased with you.
Alternatively, the Registrar can give you an access code for the service which will allow you to complete the process yourself either by telephone or via the internet.
If you wish to undertake the full service please bring the following information with you to the appointment (if appropriate):
- National Insurance number (of the person who has died)
- driving licence
- concessionary bus pass
- library card
- disabled parking blue badge
- details of any benefits being received (if appropriate)
If the deceased was in receipt of a Public Sector Pension we may be able to advise them for you.
